Hip osteoarthritis — the gradual wearing-out of the hip joint — is a major drain on quality of life, and a leading reason people lose months of working life to pain. At the far end sits a hip replacement: an excellent operation, but a big one. So there is real interest in treatments that might calm an arthritic hip with far less than major surgery.

The fat-injection idea

One of the more promising is an injection of microfragmented adipose tissue (MFAT) — a preparation made from a small sample of your own fat. A common follow-up question is whether adding platelet-rich plasma (PRP), the “blood-spinning” injection, makes it work even better. More is not always better, so it is worth actually testing.

What we did

In this study, on which Professor Heidari was first author, 147 patients with hip arthritis (across all grades, 1 to 4) received either MFAT alone or MFAT combined with PRP, and were followed for a year.

What we found

Both groups improved meaningfully in pain and function. More than 60% of patients reported a 20-point or greater fall in their pain score — a change large enough to genuinely matter in daily life. In other words, a single biologic injection bought a real, clinically important improvement for most patients over the year.

What this means for patients

For the right patient, a fat-based injection is a reasonable option to consider — particularly for those who want to delay or avoid a replacement, or who are not yet candidates for one. It is not a cure for a severely worn hip, and the evidence here is observational rather than a head-to-head trial. But it is a genuinely useful, lower-risk tool to weigh up, and exactly the kind of option worth discussing before assuming surgery is the only road.
