Limb reconstruction is a relatively small specialty — perhaps a few dozen surgeons in the UK do significant amounts of it — and that means research funding is correspondingly tight. So it matters, more than perhaps in larger fields, that we spend the research budget we have on the questions that will most change patient outcomes.

Why a Delphi process

The Delphi method is a structured way of arriving at consensus among a group of experts. You ask a question, take in everyone’s answers, summarise the patterns, and feed it back to the group to refine. It avoids the loudest voice winning. Done well, it produces a ranked list of agreed priorities that funders can act on.

What we did

The British Limb Reconstruction Society Research Collaborative — on which I sit as a contributor — ran a modified Delphi study with 115 UK limb-reconstruction practitioners. We asked them, anonymously and in successive rounds, to identify and then rank the research questions they thought would most benefit patients if answered.

What rose to the top

The single highest-ranked priority across both adult and paediatric practice was how to measure patient-reported outcome (PROM) well in limb-reconstruction patients. This is striking. Surgeons themselves are saying: we need patients to tell us whether our operations actually worked — not just whether the bone healed on the X-ray. That is the kind of insight that should anchor the next decade of work.

Other priorities in the top tier: how effective specialist physiotherapy actually is during the long recovery, how to provide psychological support through what can be a two-year process, and how to choose between operative strategies for the two hardest scenarios in limb reconstruction — infection and limb-length discrepancy.
